- Mandatory Fields: Fill all required (*) fields accurately. Incomplete forms will be rejected.
- Contact Information: Provide a valid Mobile number, WhatsApp number, and Email ID.
- Document Uploads: Prepare scanned copies (JPG/JPEG/PNG, 10KB-200KB) of:
- Student Photo
- No Dues Receipts (Department, Library)
- Admission Receipt, Marksheet, Provisional Certificate
- Aadhar Card
- University Registration Card is optional
- Certificate Issuence Charge:
- Normal: 3 days (Fee: ₹600)
- Urgent: 1 day (Additional Fee: ₹200, Total: ₹800)
- Payment Options: UPI, Debit Card, Credit Card, Net Banking
- Review Before Submitting: Ensure all details are correct; once submitted, the form cannot be edited.
- After submitting the form ensure that Self Attested hardcopy of all uploaded document have to submit to the college along with the application form.

- Transaction fee charges would not be refunded/reversed under any circumstances for any refund/reversal/chargeback and any other reasons.
- Transaction fee charges would not be bourne by cardholder for any payment.
