1. Eligibility: Only for B.A./B.Sc. Part-III pass-out candidates of Gaya College, Gaya.
2. Mandatory Fields: Fill all required (*) fields accurately. Incomplete forms will be rejected.
3. Contact Information: Provide a valid Mobile number, WhatsApp number, and Email ID.
4. Document Uploads: Prepare scanned copies (JPG/JPEG/PNG, 10KB-200KB) of:
   • Photo, Signature
   • No Dues Receipts (Department, Exam, Library)
   • Admission Receipt, Marksheet, Provisional Certificate
   • Aadhar Card
   • University Registration Card is optional
5. Certificate Issuence Charge:
   • Normal: 3 days (Fee: ₹368)
   • Urgent: 1 day (Additional Fee: ₹150, Total: ₹518)
   • Payment Options: UPI, Debit Card, Credit Card, Net Banking
6. Review Before Submitting: Ensure all details are correct; once submitted, the form cannot be edited.
7. After submitting the form ensure that hardcopy of all uploaded document have to submit to the college along with the application form.
